PURPOSEOF CLASSIFICATION

The purpose of this classification is to provide information, direction, and assistance for Health Department patients/clients and their families experiencing difficulty in meeting their medical and/or socio-economic needs. Duties and responsibilities include interviewing and counseling patients, conducting patient assessments, and making referrals, performing follow-up, interacting with other community service agencies, preparing documentation, maintaining case records, and performing other duties as assigned. Reports to the CHANT Supervisor and the County Health Director.
ESSENTIAL FUNCTIONS
The following duties are normal for this position.  The omission of specific statements of the duties does not exclude them from the classification if the work is similar, related, or a logical assignment for this classification. Other duties may be required and assigned.
Provides case management services for eligible patients/clients and their families who, due to medical and/or financial situations, find it difficult to meet their medical and/or socio-economic needs; works to ensure needs of clients are met and to reduce duplication of services by other agencies; for example, Children Special Services (CSS).
Provides specific program services as required by applicable guidelines and regulations.
Establishes rapport with clients to build trust and encourage participation in services.
Works with other providers in the department and community to ensure that patients/clients receive all available services.
Works closely with community agencies to ensure awareness of Health Department programs.
Provides necessary referral information.
Completes large volume of required paperwork accurately and in a timely manner.
Submits paperwork to a variety of agencies, offices, or individuals.
Participates in community outreach events to promote CHANT services and engage underserved populations.
Interviews patients/clients to determine immediate needs and appropriate services to meet those needs.
Assists families in prioritizing their problems and needs.
Discusses identified problems with patients/clients and develops a mutually agreed upon service plan.
Establishes realistic goals and timeframes.
Explains programs/services to patients/clients.
Maintains awareness of services/resources available within the community
Makes referrals to community agencies as needed.
Completes related forms/paperwork.
Performs follow-up to ensure patient/client referrals are completed and needed services are obtained.
Conducts home visits as required within specified timeframes.
Accepts referrals from department personnel.
Serves as a resource for department personnel seeing patients with social service needs.
Prepares complete, accurate, and timely documentation of each case. Places documentation into medical records or other files as appropriate. Maintains confidentiality of all patient/client information.
Ensures that any release of information to other providers or agencies adheres to policies/procedures governing patient confidentiality.
Prepares or completes various forms, reports, correspondence, encounter forms, case documentation, agency referrals, service plans, or other documents.
Receives various forms, reports, correspondence, case records, medical records, referrals, policies, procedures, manuals, reference materials, or other documentation.
Reviews, completes, processes, forwards or retains as appropriate. Operates a computer to enter, retrieve, review or modify data Verifies accuracy of entered data and makes corrections.
Utilizes word processing, spreadsheet, or other software programs.
Performs basic maintenance of computer system and office equipment, such as backing up data or replacing paper, ink, or toner.
Communicates with the supervisor, employees, other departments, patients/clients, the public, medical providers, social service providers, community agencies, government agencies, and other individuals as needed to coordinate work activities, review status of work, exchange information, or resolve problems.
Maintains a comprehensive, current knowledge of applicable laws/regulations.
Maintains an awareness of new trends and advances in the profession.
Reads professional literature; maintains professional affiliations. Attends workshops and training sessions as appropriate.
Maintains a clean workspace.
Performs Emergency Preparedness tasks when needed in various locations countywide providing preventative care within the community
Assists in setting up and taking down Public Health Points of dispensing as needed.
Must work hours as scheduled by the supervisor or county health director, which may include weekends, after hour events, and holidays.
Complete work at assigned workstation as determined by supervisor.
Completes any reasonable duties assigned by the supervisor.
ADDITIONAL FUNCTIONS
Operates a motor vehicle to conduct work activities as needed.
Performs general clerical tasks, which may include answering telephone calls, typing documents, making copies, sending/receiving faxes, maintaining current reference materials, filing documentation, or processing incoming or outgoing mail.
Provides assistance to other employees or departments as needed. Performs other related duties as required.
MINIMUM QUALIFICATIONS
Education: Bachelor’s degree in social work, Sociology, Human Services, or a closely related field.

Substitution: An Associate’s degree in a relevant field and two (2) years of related experience may substitute for the bachelor’s degree requirement. Experience should include previous work and/or training in social services, human services, or case management.
Equivalent Combination: Any equivalent combination of education, training, and experience that provides the necessary knowledge, skills, and abilities to perform the duties of this position will be considered.

Additional Requirement: Must possess and maintain a valid Tennessee driver’s license.

Physical Ability: Tasks require the ability to exert very moderate physical effort in light work, typically involving some combination of stooping, kneeling, crouching and crawling, and which may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (12-20 pounds).
